We have an exciting opportunity for x2 full time positions for the post of Health Check Officer:

HBC3 £19,650-£20,043 per annum
Halton Health Improvement Team are looking to recruit Health Check Officer’s on a fixed term contract to work as part of an innovative team supporting individuals in the community to make healthier lifestyle choices to improve their health and wellbeing.
You will undertake NHS Health Checks and Personal Health Plans connecting individuals with mainstream services in Halton, specifically around: exercise, healthy eating, building confidence and self-esteem, social activities, learning new skills, drugs, smoking and alcohol.
You will work well within a team and should have excellent communication, interpersonal and customer service skills to create good working relationships with key local people (including frontline staff) and community groups.
A good working knowledge of the local area, an ability to work under your own initiative and the ability to embrace change is essential. You will be well organised and be able to successfully manage a case load of clients in an efficient and effective way. Due to the Covid-19 pandemic many of our services are operating in different ways and an ability to embrace working remotely and be flexible is essential.
You will be required to apply to the Disclosure and Barring Service (DBS) for an Enhanced DBS Check or an Enhanced DBS Check for Regulated Activity. Further information about the DBS Scheme can be found at https://www.gov.uk/disclosure-barring-service
The role will involve regular travel across the borough and sometimes further afield. Therefore, the postholder must have a driving licence and access to a vehicle. Where necessary, reasonable adjustments will be made in accordance with the Equality Act 2010.
