Halton Borough Council is offering a unique opportunity for an experienced Social Worker to become part of the Hospital Integrated Discharge Team (based in Warrington Hospital).

Halton Borough Council’s expectations are high and you will need a strong commitment to providing first class person centred care to Halton residents over 18 and their carers.

The Social Worker role involves discharge planning, working within the timeframes of the Care Act. You will need experience in completing assessments and planning to facilitate safe and timely discharges. You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties), whilst challenging discrimination and promoting independence.

You will have knowledge of  relevant social work legislation such as the Care Act, Capacity Act etc and a good understanding of the principles.  You will also have knowledge and some experience in risk assessments and risk management.

You will be a committed team player, dedicated to Multi agency team working within the hospital setting and be able to manage and organise your own case load.

You will have a strong personal commitment to anti-oppressive practice and be able to demonstrate your commitment to challenge discrimination through your daily practice.

You will have good interpersonal and communication skills, a flexible approach and a willingness to learn new skills and continually develop.  Ideally you will have good IT skills.  In addition you must hold a full driving licence.

In return you will receive a car user allowance and a flexible working week. You will receive up to 33 days annual leave per annum and a commitment to your on-going development.
